How to identify the loss of a sense of reality, in a Boderline personality disorder ?

He is extremely violent and unable to control his anger. He cut his wrist 2 times (not deep but still it happened within the same month). Don’t know how to recognize this symptom though. I must be able to notice how he behaves when he really looses sense of reality.
Its almost next to impossible to take him to a psychiatrist so please help me with this.
